You should contact BMC support then.
But it's weird, why does it ask you for the database? I looked at my installation document and it only asked for "Demo" user, never for database credentials.
I always checked everything BUT unchecked those three "service level management collector" and "optional service level managment AR Integrations".
We have checked service level management collector during installation.
May be due to that its asking to enter DB details.

In this doc I found known Issue:
When you install the BMC SLM Collector in an Oracle RAC system, there are required steps for installation and configuration which can be found in KB article #20038295.
But I could not find that KB article.
There is a KB here about SLM and RAC, looks like your problem:
I couldn't find the KB as well, I guess since you have the defect ID you can contact BMC support and ask for the KB reference.
