Yes; I recommend searching on "SRM Survey" on this forum; that should get you a lot of info. You can also consult the SRM manuals on support.
You can configure survey from app admin console --> custom configuration --> Service Request Management --> Survey configuration.
After survey is configured for Company ,you can attach this survey to SRD.

Ah, well, if you are using RemedyForce the answer is different. I'll move this to the Remedyforce forum.
With RemedyForce we don't include a survey with the tool; instead we recommend using one of the survey tools from appexchange such as Zoomerang or SurveyMonkey or one of the others.
