You declared the form in the RKM console as a "Knowledge source" in "Manage Knowledge sources"?
I tried this on my VM 7.6.04 SP2:
-> I created a regular form "_lma_test_template", added public permission.
-> Added a "GUID" field (179) since it's mandatory.
I went into "manage knowledge sources":
-> added a "Knowledge Base Item", "AR Form",
-> I chose "_lma_test_template" and didn't fill "view name", and I selected "indexing" interval to "2 minutes",
-> I chose "Description" field for "Title", "Keywords" and "Environment",
-> I chose "map to existing fields",
-> I didn't choose anything in "Organisation", only added "- Global -" "All" as visibility group,
-> nothing on categorization,
-> On "content field" I chose field "Description" to be indexed,
-> for accessibility I chose "- global -",
In your case, did you select any fields to be indexed at the step I've highlighted in bold?

I have set some fields in relavancy field shows in the attachment. but still throwing an error.

AFter getting this error,DB transaction begins and commits successfully,but console shows error.
Error showing either form list is empty in our case it is 2970 as per screenshot in my previous mail.so can you please tell me what wii be the cause for that error.

It's actually in "Content field" that you define the fields you want to index , so 4 steps later, do you have some selected?
This issue is got resolved after disabling the workflow which is setting default value to company name field used in the templete field.
