First of all it could be related with the KM-000001007502. The problem might be with following filters:
I believe that this is a known issue .You may contact with BMC support and they shouold be able to provide you correct def files for the filters.

I have gone through KM-000001007502, and imported DEF file, which is given in this KB article, but all 3 Fitler Guides already exist in Remedy.
I have also enabled Filters, which are mention in KB.
But still I am facing same issue.
Delete all records from CAI:Events form where status is Error and then restart caieventcmd plugin.
Does problem still persists ?
Status does not have option "Error", but Return code field has option "Error", hence I deleted all records which have return code Error.
Then, I dont know how to restart caieventcmd, hence I have restarted AR Portmapper and then AR Server service.
But still I am facing same issue,
Sorry for late response. We are planning to upgrade to Remedy 7.5. So this bug will resolve in this upgrade. Thanks for your valuable time and response.
We have resolved this issue by creating filter which will assign defula Operational CTI for each request created from requester console.
This has resolved our attachmenet issue.
