Please check via debug. It can be a SOQL101 as example. Maybe from a recent process builder.

Could also be permission issue if its only for a specific User.
Please share any screen shot if you can, else contact BMC support to help investigate further.

Thank you Amit. As it is intermittent (even on the same ticket) I had ruled out permission issue. Sorry about lack of screenshot, I had pasted it in but obviously didn't publish it.
I'll contact support if I have no luck with the de-bug Paul suggested.
We use templates. It is only effecting 1 user and isn't consistent.
I had wondered if it was linked to the compatibility issues with Chrome but given it is intermittent I suspect not. To be safe, I have asked him to try in Firefox to see if this replicated at all.
