In the doc you linked, it says that the attached license (Smart Reporting 1808) will expire on 31/12/2020.
After that date will another license file be available to download?
We are using AR Server version 9.1.03, which license file should we use? Smart Reporting 1902 or Smart Reporting 1808?

You will have to use the License file of SmartReporting 1808 since your current version is 9.1.03. This variance is due to the build version of Yellowfin product. Yes, Yellowfin is the underlying skin of SmartReporting! :-)
Coming to the licensing aspect as Vinayak mentioned, SmartReporting doesn't come with additional cost for you and is free with Remedy AR Server product.
There is a need to manually import the NEW license file before it expires because of the Yellowfin policies i.e.it releases the new license in every 3 years . However, BMC will share the SmartReporting license with its customers with no additional cost.
Hope this provides some clarification to you.
One last question,
In this doc: BMC Remedy Smart Reporting without ITSM license it says that without an ITSM license you cannot use the Smart Reporting component.
I can confirm that we have an AR Server License and a key to go with it, but I'm not sure about the ITSM part, are they the same thing?

Remedy AR System is the platform and Remedy ITSM is the application that runs on the AR Platform. They are not one and same.
Talking about License aspect you would need AR License along with ITSM (Service Desk) Fixed/Floating licenses too.
Having said the above, yes, it is mandatory to have the ITSM installed to be able to leverage the free product Remedy SmartReporting.
Though, after hearing the request from customers to support Custom applications apart from ITSM, BMC have added this functionality starting 18.08 release. You can find more details in the below link:

I've got other doubts,
In the BMC docs it is recommended that one should install the Smart Reporting server on a machine different that the AR Server, in that case, do I need to install an AR Server/ITSM Suite on the other machine as well or the reporting component works stand-alone too? (In that case we will have to install an external tomcat as well right?)
If we have to install the AR Server / ITSM on another machine, we will need another license if what I'm thinking is correct.
Another silly question, how can I check if I have the ITSM installed on a server?

For POC you can install all the components of Remedy Stack on one standalone server which has adequate resources to cater the minimum MEMORY/CPU/HARD DISK that is required by each component.
Usually AR System & ITSM Suite are deployed on same machines and the rest applications like Mid-Tier, SmartReporting, Smart IT, DWP, RSSO etc will be hosted in a different servers to well balance the load and as well to avoid the single point of failure.
It is obvious that it will compel you to install bundled/external Tomcat for necessary components that requires Web Application (Tomcat) on each of these boxes. Though if you are looking for POC then you can leverage the Single/Multiple Tomcat instances on same machine to cater all the components.
Note: Tomcat Service name has to be unique for each instance to avoid issues.
