I was able to reproduce this and it seems in database we only save it once, but in UI we display it the number of times it exists in the named list.
I will try to check internally if we can create a defect for this.
It seems it only happens if you select in the named you do not return the ID but the label name.

Can you tell me which named list you used? Are those coming from BMC?

Those are all custom named list's.
I did tried with storing ID or GUID, but then you would still need to maintain extra field to store value as well especially if it used in Record Grid sorting and filtering. There are multiple fields like this with different named list's on the same record (and some are in select group), so I would need multiple fields for values as well. In reality we are after values in those fields. Additionally, source records for those names lists can change not daily, but often, so no point gluing ID's or perusing and maintaining associations.
I believe same approach with data driven menu's in Remedy was always showing distinct list.
I figured work around where I will have to restructure source records vertically, but in reality would be nice to have distinct as an option for those named lists
