I don't know of a way. Just because they have the same name doesn't necessarily mean they are the same attachment, so not sure how this could be done.
Yes, the first set image001, image002, image003 are different. I want to prevent the duplicate sets. Thank you for the feedback.
I agree with Anne. Sounds to me that even if you have the same name, same extension and the same size, still you cannot tell any code that it’s exactly the same content.
Sorry, only way is to except that risk, otherwise I would say add a trigger to delete the ‘duplicate’ when above criteria are met. Be aware that an auditor may not really like it.
Assume that it is the same content including file type, is it possible to do a validation rule, or a check with flows, apex, or trigger? If yes, which object do I need to look into? Thank you, appreciate the feedback.

I posted an idea to provide admin the ability to filter out duplicate attachments.
Here is the link Ability to Filter Duplicate Attachments from Email Conversation.
Thank you BMC Helix Remedyforce!

I don't know if RemedyForce has this capability, but the only way to verify that two image files are exactly the same is to check the MD5 checksum of both. If it matches, it's the same image. You would need a way to create a filter that verifies image checksums and then filters out duplicates it finds.
To be clear these are the same attachments that gets reinserted to the same record from a response to the same email thread conversation. In addition, I checked visually that they are the same images with the same time stamps from the same email conversation history thread. I will reopened my case with support to see if this is a bug or a defect. Thank you all for your help.
