There is no way in FP11 to know which approver belongs to the team.
You would need two approval processes to trigger in sequence - one for each team
This can be done easily in V12, fwiw You could use two different State Fields

I have one more question in change manager.
I have added my self as eligible approver,so i could approve the phase.
At some point,i just want to edit & add description and I don't want to approve/disapprove/defer
But this is not happening. when i just want to add description ,it is not allowing me to save.It is also expecting me to approve/disapprove/defer.
Is there anything to do with?

I have set force mandatory comments for Approve,Disapprove and Defer.
When i want to do any change management activities ,i can go into change management tab and do approve/disapprove/defer with necessary comments.
But now i don't want to do any change management activities. I just want to add description .
when I click edit and add description & try to hit save ,now the change management tab automatically set to defer and asking me to enter comments for defer.
If the user is not eligible approver then they could add description without any issues.

Your configuration is saying that you need to put a Mandatory comment at this point.
You will have to put a comment in the Defer comments box.
At this point in the ticket lifecycle the agent is not in a position to vote - so they must be deferring the vote until later.
For this reason they need to put the comment in explaining why.
I don't want to defer and I don't want to do any change management activities.
I just want to add description or any field update
I understand but by definition, you are not voting approve or disapprove at this point so you are deferring.
You have it configured to ask for a Comment for defer, so you need to add one I'm afraid.
We cannot make the option Mandatory and not ask for a comment.

I have some doubts. I have created two approval process for 2 different team.
For Eg :-
Condition for Approval Phase 'Security' : Security = Yes
Condition for Approval Phase 'QA' : QA= Yes
I have created a RFC with both Security and QA as Yes
My expectation is both two approval phases should be triggered based on conditions
But this is not happening.
