I don't have any idea how to insert value in the service url field, the link said use like this format.
But when i tried to changed the cs57.salesforce.com with my domain, it shows blank url.. should it show the incident form of my environment?
Can you please give me the detail example of it.

Please try following-
Create new profile, give permissions only for Incident object and API, then connect with the same user using SOAP API.

The Soap UI response is success when send the request for create new account.
But when i tried to search the records in accounts forms, it's not showing any records. Do i missed something in the steps?

If this setting is checked, then as part of your API, if the account is intended to be used solely with Remedyforce, you'll need to ensure that the Remedyforce Account field (BMCServiceDesk__Remedyforce_Account__c) is being set to TRUE.
Yes, the settings is checked.
then i checked Remedyforce Account field (BMCServiceDesk__Remedyforce_Account__c) i couldn't find the set true properties, but i assume is already set true for that fields by default. is there any configuration that i need to checked so the accounts can be created through the API? but it's odd the response from the soapui was success
How specifically are you looking for the Accounts via the user interface?
