Incident History schema (or any object) can be seen from setup area->Objects->Incident History-> Fields and relationship:
The Description field stores the summary of the action and Note field stores the detailed information - such as note entered by a user, or the email body when email is sent or received.
For the lookup information, the same screen gives you the object on which the lookup is created for in ()
If you don't see BMCServicedesk__description__c or BMCServicedesk__note__c etc using API, then it means the API user doesnt has field access on those field. Please check with the system administrator of that org to grant the read permission on all fields you want to use for integration.
Reuben is able to login to Salesforce through the standard method and run Salesforce reports to view the contents of BMCServicedesk__note__c (please shout if this isn't correct Reuben). Though he doesn't see the field when trying to login through the API with the same Salesforce username. What could be the reason for this problem?
The first thing should be checked is for that user who is logging via API is having Read permission on that field and the field is marked as visible.
This can be checked for the User's profile.
Hi Amit, the user logging in through the API is the same user logging into Salesforce directly. The user can see the field through the Salesforce ui, but not when logging in through the API. I have checked the permissions for this user and they have read access to this field.

I think the issue you are saying of field not visible is only on Tableau? I assume if you use tools like Data loader, the same user will be able to see those fields - can we check.
There seems to be a limitation on Tableau side where tableau doesn't show Long Text Area fields if their size is more than 4 KB. Please check the Cause 3 in this KB: https://kb.tableau.com/articles/issue/salesforce-fields-do-not-appear-as-expected
Yes that's correct (or at least that's my understanding of Reuben Lewarne problem. We use tools like data loader, but Reuben wouldn't have access to this. Though he could use workbench and try to query the object. I have been able to, and can see the note field.
