You can probably reach out to Support and they should be able to assist.
I'm not sure what the format is to get to Submit a Ticket form but if you figure it out, please share.
However, there is a workaround.
1) create ticket Template - something generic like 'new ticket'. doesn't need any actual values set, unless you want to set the Queue.
2) add to 'common tickets' panel
3) any common tickets in the SSP have a URL. The format is:
*DOMAIN is your custom domain/instance name.
*id= should be the salesforce id for the newly created template

Thank you Joshua and Kedar for the suggestions. We actually found what we were looking for under Remedyforce Administration, Configure Self Service, and Tile Visibility and Order. From there, you can copy the direct link to SS tiles:
We just modified this link to put our 'my.salesforce.com' at the beginning and it worked like a charm!
