You can create 2 custom fields for process level and job grade in Incident object.
Then using fulfillment - select template and map the fields.
This will store the data for reporting.
Note - this will give you report for all future records.

Please try with report type title "Request Details with Request Detail Inputs and Fulfillment Inputs". From this report type you can filter out what you need as in a specific service request and or an Incident Number (from the Request Detail section). From here you would need to display the fields as needed for your report such as: Input/Prompt, Response, or Stored Value (from the Request Detail Inputs Section). I find this convenient for one off report request and not creating unnecessary fields on the object.
