Work Log is nothing more than another Regular form with a few fields on it....the parent record and the work log record are typically associated with a guid....
The incident form is showing a Table field that lists the existing work log entries, and various workflow that allows you to see the contents and add new entries...the workflow that fires on 'add note' does a push to the work log form and likely refreshes the table after it's been inserted so you can see that it's been done...

Your qual is all wrong.....
The Run-If Qual for the AL should be 'z1D_Note' != null
The Push Qualification should be null
and your 'any requests match' should be set to 'do nothing'....
That will make the AL only fire when the note isn't null, and will always create a new record in the form in question.
