Do you see your request in SRS:RequestApproversLookup form? Request Number = SR ID
What is your AR version?
seems like mid tier issue
there were some issues with approval rules in 9101 server which were resolved in 9102.
However as you have 9.1 it should not be problematic.
Do you have user tool? There are some issues in approval rules form if we update from mid tier.
User tool would be safe.

Do you have entries SRS:RequestApproversLookup form?
I'm also facing the same issue. Please find the details below:
1)We have service request where we need approval defined based on the user response.
2)In the service request, have mapped the question to the field SRM_field_Type_35 (where user resposne is captured)
3)In service request changed approval from None to CUSTOM
4)Created approval mapping record for the group "XYZ", members of XYZ group have the Request Approver functional role.
5)In Approval chain form, created new record for the service request.
6)Created SRM:AutoApprove record for the service request.
7)In Ap:Adminisrtation form enabled SRM:AutoApprove record which was in inactive status.
8)I don't see any record created in the AP:Detail form for the service request raised.
9)However I can see record in the SRS:RequestApproversLookup form. PFB screenshot
Now when user create a request then approvals are not getting attached to the service request.
