So the issue is Intermittent- It works for some scenario and not for some.
Server logs (filter+SQL) should help on this. Try capturing for both working and non-working scenarios.

This is skipping filter to order 770. What is the qualification of this filter.
Disable this filter and test.

The non working logs do not show the activity when escalation is run on the form NTE:SYS-NT Process Control. So I cannot find any filter running on NTE:SYS-NT Process Control in non working logs. May be you need to capture non working logs for longer time when records are processed from NTE:SYS-NT Process Control form.
I have copied the logs after seeing an entry in AR System email messages and verifying the data in it. Somehow cannot find corresponding log entries.
The issue has been resolved now!
My dev environment has two AR servers in a server group [Primary and Secondary], there was another AR server pointing to the same DB which is not part of the server group.
The notification engine used to somehow pick this 3rd AR server causing this intermittent issue with the email notification. I have stopped the 3rd AR server and the issue is not occurring any more.
