Do you have fresh install of 9.1.x or upgraded the existing 8.1 environment?
I hope you have followed the best practices to upgrade the environment Remedy 9.x Upgrade Enablement follow the best practice link.
Note:- Never apply changes on the base development mode, you should apply the customization on best practice mode.

If you have fresh installation of 9.1 the the easiest way is to make HPD:Help Desk Overlay and then add the custom fields.

Your 8.1 customisations were not done using overlays? Depending on the scale and complexity of the changes you have made there are a number of options:
1) take a clean install of ITSM 9.1 and re-create your custom changes as overlays and then migrate your data
2) convert your changes on the 8.1 system to overlays and then upgrade
There are a lot of factors which will need to be taken into account when deciding what to do. Make sure you take some time to look at the docs and the many online resources available to help with your upgrade planning.

We have opted to manually create the custom fields via overlay for easier migration from development, uat, and production environment in the future.
