Probably because it’s per-instance cleanup which only affects the directories in the instance’s tmp directory.
But it is necessary to cleanup other temporary directories under ./NSH/tmp which begin by [0-9] older than 3 days.
It creates message in appserer.log such as "Too many temp directories in /opt/application/sgaapp/current/NSH/tmp/"
So I am going to create a little script to do this action, with someting like that :
find <BSA_install_dir>/NSH/tmp -type d -mtime +3 -print | grep "^<BSA_install_dir>/NSH/tmp/[0-9]"

Yes, it is needed to cleanup that directory periodically. Just make sure to stay one level deep and don’t delete the appserver tmp dirs.
It would be possible to create a idea to include in BSA cleanup a script to answer to this need :
