    How do you create responsive Service Request forms?

      I don't see a way to create form specific picklists, text boxes, radio buttons, etc. in order to capture user input. I'm comparing this to Remedyforce where you can configure this on each individual request. It looks like you would have to configure a list of questions and place them in the description via a template in order to capture specific information but this wouldn't allow you to give the user options to choose from. In addition, it looks like you could configure the form with picklists, etc. but this would display on every service request where the questions wouldn't typically be the same across all forms.
