Check if the Company is active on Company form, and check if its pushing correct company name to this form.
Also check if the Company was configured correctly on People form
We had the same issue today.
I checked the forms SRM:Request and CAI:Events. In CAI:Events I found the entry "HPD:IncidentInterface" in the field "App Interface Form".
I opened the HPD:IncidentInterface searched my INC and filled the field "Company" (which was required but empty).
Retry in SRM:Request and it worked again.
