This is only stored in 'Change History'....and only if someone enters change history. The alternate, depending on if you have it configured that way is if you have revision history configured, Remedy automatically saves a copy of the object in the history form for each modification, when and by whom, and optionally with an export of the object that can be used to go back to that revision.
Other than those two, which are optional, I don't know of anything.

Interesting. How would I check if I have revision history configured, including with the optional export of the object for reverting to that revision?
In one such H table for a given form, I see entryId, and several Tx and Ux (where x is an integer) columns but I don't see a column for, say, field id or something which I can use to identify if a field or anything on the form was modified.
I suspect those are the only two methods. I appreciate your guidance, thank you.

Addition to LJ LongWing explanation, you can only find the last changed and timestamp of change in database easily.
for example: ACTLINK table stores the active links, if you loot at this table, you will see TIMESTAMP ( last changed or created date stored in this field), LASTCHANGED field holds the login id who last changed it.
Both fields are pretty much in all the objects tables. i.e ACTLINK, FILTER, ARSCHAMA, MENU ....etc.

Login to System as Administrator and navigate to Applications --> AR System Administration --> AR System Administration Console --> System --> General --> Server Information on Version Control tab.
Enable and set Save definition files will create backups.
Thank you, Mohammad. Yes, Last Modified and Last Modified By are no issue. I mean to say those are quite accessible. I was looking more for an audit history of workflow changes, i.e. if a piece of workflow was modified 10 times, what are the modified date and time values for each of those 10 modifications?
Real world example:
I have to modify a number of records in a given form. Specifically, a selection field for validation, e.g. "What was your first car?" There is a corresponding character field to fill in the answer. I have one form in two different environments and each with different modification date time values. I'd like to see the modification history of the form in each environment to try and align the changes. The question field in env1 has a number of selection values. One such selection value is "What was your first car?". It has an ID of 1. In env2, that same value has an ID of 2, but I know the ID used to be set to 1. I just don't know when the change was made. I now suspect I have an answer tied to the wrong question for a number of records, as a result. If I could compare the modified date history of the form from one environment to the other, I might be able to determine when the selection field was modified. Then I can isolate the records based on before and after the workflow was updated. I want to ensure I'm modifying the proper record set.
As I mentioned earlier, you need to enable the version control. Using the object modification log - Documentation for Remedy Action Request System 18.05 - BMC Documentation .
I am not aware of other ways to find out, i am sure experts will shed the light if there is hidden ways to figure it out.
You are looking for auditing on 'metadata' changes and it is registered in 'AR System Version Control: Object Modification Log' form if version control is enabled on the server. If it is not, you can only get LAST modified entry.
