So....the Work Log is associated with the Ticket....so, you can fire a Filter on the submit of the worklog, look up the integration id from the incident, into the worklog, and then use that as your input.

it could all be done in a single filter, but I would recommend two.....one to look it up, the second to perform the integration if the data looked up exists

Well LJ as already given you correct answer, I would elaborate little more on same point, on submission of worklog you should have first filter that will perform look up on Incident form for SNOW ID and set the same on worklog form.
Second filter will trigger after first, which will check if SNOW ID is not null and then will consume SNOW webservice and push worklog details.
