Seems like an older version, but I reckon its Org Ref Code.

When I select Org Ref Code from the Format drop down menu the Reference Type field is empty.

I have made a little progress but not sure that's what you wanted to display the report, I have added the MonthC org ref code using "Last Resolved Date" for Closed tickets, Month is used for Submitted and Open Tickets, But I am not able to use the Overlay Combination Chart to display as you have shown because of the measures. I hope experts can chime in and help here or give BMC Support a Buzz.
I am not an expert but learning from community as well.
There are two ways to get the report. Using the Submit date for created, open and closed and second is to use submit date for created and open and use Last resolved date for closed in same month. using submit Date makes it little easier but closed using last resolved date will add one more union.
Final Report ( Using Last Resolved Date for Closed):
Calculated formula for create tickets.
Step 2: Add Union for Open Tickets
Map the fields see next screen shot. create calculated field open tkts and use for sub query filter to only show open tickets.
Repeat step 2, add union for Closed tickets and add calculated formula for closed ticket.
Note:- If you use Submit Date then no need of this step, in previous calculated field you can open and closed in same formula.
After saving, you can click on the advance function of Filters and make sure they only show the open, closed for union 1 and union 2.
Unfortunately, i can't be big help and I am sure Smart Reporting Experts will help you achieve what exactly you want.

The table doesn't have multiple columns\rows for same month year.

This post was extremely helpful as I am trying to create a similar report. However, even after following the steps in the word doc I am unable to accurate results for my resolved data.
The only difference is that I am creating my calculated fields off Submit Date & Last Resolved date. Basically I want to see how many tickets were submitted compare to resolved in the same month for 2017. Below are some screen shots... is it possible to create this report? If so, what am I doing wrong? I have confirmed that the data for Submit Date is correct, but my resolved data is inaccurate still.
For the operator option I have tried Equals, Equals Date Component & Same Operator, none of those options are giving me accurate results.

Try with these changes into the report and see the counts.
1. Remove the filter for Last Resolved Date form the report and keep only Submit Date as filter.
2. Create a calculated field for Submitted Count as shown below -

4. Go to Advance Function for Count Submitted field and set "Null to Zero" from Analysis function. Do the same for Count Resolved as well. This will help is displaying blank/null values to Zero.
I verified the counts and giving correct results.
With the changes, the report will give counts for the incidents submitted between the period given as the Date filter, and out of these submitted incidents only it will show the resolved counts.
Here is my report output.

Thank you for the quick response, however, this is not exactly what I am looking for...
The Submitted calculation is only giving me those tickets that were Submitted in that month and are still open. I want to create a report that will look at ALL tickets (regardless of status) that were Submitted vs. how many were Resolved for the same month.
Here are the results I received using the above calculations:
1st Report: Month Name Submitted; Count of Submit Date (Filter: Submit Year)
2nd Report: Month Name Resolved; Count of Last Resolved Date (Filter: Resolved Year)
When I create 2 separate reports (First Report: Submitted; 2nd Report: Resolved). These are the numbers that I am expecting to see. So basically I just want an overview for each month in 2017 where I can see ALL the tickets Submitted vs. Resolved (regardless of existing status). is this even possible?
