The service request fullfilment is done by CAI plugin.
There are some CAI configuration forms which you need to check.
As far this error is concerned, it means that you haven't registered your AOI interface form in CAI:AppRegistry form.
Please follow below steps:
a) Open CAI:AppRegistry form( Custom Configuration --> Foundation --> Command Automation Interface form)
b) Search for Software Self service application in this form.
c) Make sure that "AppInterfaceForm" field on this form for this application is populated.If it not populated then make Instance form field value same as Interface form.
d) Save this record and try to create new request.
e) If this doesn't solve the issue then take server side filter logs to check the filter which is causing this error.

Which backend fulfillment request are you creating via SR ?
I can't find any records for Software Self service application in CAI:AppRegistry form.

As per the above screen shot, you are at the Purchase Requisition AOT in the process of that SR and somebody has entered the workinfo in SR.
The workinfo is not travelling from SR to PR that's why you are getting this error.
If this is the case then let me know.
