Ootb means out of the box. It means that it is an object from Bmc, for exemple hpd:help desk.

I have deleted the Regular form created by me.
I don't see how you could retrieve that information.
Check in your database if you still have an entry in arschema table about your form:
select * from arschema where name='yourformname'
but I doubt that.
Else perhaps you created a special user and gave him permissions to access it? You could look at the user's permissions then.
If you've deleted the form and don't have a copy in a def file or on another server, then your only option is a database backup of the ARSystem database that you would have to restore. If you don't have that then you're out of luck until someone invents a time machine.
You would need a copy somewhere...but being it is a form you created....one would assume that you set it up, so you should have a pretty good idea how it was setup, right? Baring that, are you on a version of Remedy that has version control capabilities? If so, you can check to see if it exists in the object modification log...or as pointed out...do you have a DB backup...maybe an export of the entire system you took at some point....
really there is no safeguard to stop you from shooting yourself in the foot as it seems you have done here

You can activate "object modification log" in server information windows, should be in advanced tabs (?) or server events, don't remember. If you didn't activate them before hand, no luck anyway.
