Did you drop the State field onto the form then save it using the save button in the ‘toolbox’ ?

Are you sure the Live form is the same one that you edited ? The title at the top of the form should match.
Try opening the form and hitting Ctrl-F5 to refresh or clear your I.E. cache and reopen the form.
That was one of the first things i thought of .....
The Field still doesn't appear on the Incident form (I have double checked it is the correct form)
Are you opening the form you edited directly from the Navigator bar, or are you clicking on a record in a quickview ?
Going into Sys Admin> Customization Wizard > Forms > Primary Application Forms > Modify > (Here I modyfy the form.)
Then I clear my cache and relogin and choose the Incident management module (from the Navigator bar)
I click incident which displays the form I have just modified but the field is missing. If I go back into modyfying the form again the field is there.
You can see why this is causing confusion...
Can you post screen shots of them both?
When you clear your cache, make sure you un-check the option to "preserve
trusted site data". As long as that is checked, your customizations will
not show up right away. This assumes that you have your SDE server set up
as a trusted site.

Either of the following are the root cause of your issue and solve the problem.
- As US mention check the permission of the newly added field in propeties. And make sure the login user belong to the same group.
- Flush the MT cache from MT configuration and relogin to the server.
- If your using 7.6.04 and above version of ARS and if you have overlay of the incident form then make sure you have added field in the overlay form of the incident.
