Self Service Ticket Forms

It would be very beneficial to be able to create different self service customer facing forms based on request types. For example, there could be a drop down field on the self service side that would back end to self service ticket form templates, so that the layout that the user sees would change based on their selection. As one example, we would like to have a separate self service form for user access requests and a separate self service form for general troubleshooting requests among many other areas vs having everything in one standard form. Ideally, you could customize the design of the self service form templates similar to how you can with the technician form templates as well.
