Custom fields for Change Management Form

Hi, I would like the ability to add custom text fields and drop-down fields to the Change form.  For instance, it would be nice to have more information about the change. Like Description of the change, frequency of this change, risk, potential impact, customer impact, expected downtime, if the change was successful or failed, back out plan and communication-- oh and a whole other section for the Emergency type of requests.


Also, I would like to have the ability to "back out" of an approved request.
