On SS3.0 when users are experience missing icons is a permission issue with the "BMC Service Request Icon" folder.
To modify the access to all users follow these steps:
1)Go to + (tab) then click on Documents object
2)Select folder "BMC Service Request Icons"
3)Click on edit link beside folder name
4)Select read/write option to all users. If you want to give permission to specific user then you can select the user from list.
NOTE: If using a custom profile the profile must have Read access the Documents object in the Standard Object Permissions and the User must be assigned the Service Desk Client proifle.
